Position Title: RN Case Manager

Department:Case Management
Position Description: To assess, plan, implement, coordinate, monitor and evaluate the care and service of patients
across the illness continuum.

    Qualifications:
    Education:GED, High School Diploma, College
    Experience:Experience in an acute care hospital setting with at least 2 years Medical/Surgical experience, with direct experience in Utilization and Case Management ~ One to two years hospital Case Management or related experience.
    Special Skills:Computer skills required: Meditech, Word, Excel, and PowerPoint. Must be able to work as a team member. Also, must be able to work independently.
    Physical Requirements:Generally sedentary, occasionally extended periods of walking required. Able to life 30 lbs.
    License Required:RN, BSN preferred, Graduate of an approved School of Nursing and currently hold a valid license in the State of Mississippi
