Social Services A social worker is available to assist patients and their families in making arrangements with long-term care centers, home health agencies, durable medical equipment providers, special treatment centers and various other public and private agencies for post-hospitalization services. Social workers are trained to help patients and family members deal with financial, social and emotional problems that relate to illness or hospitalization. Social workers offer assistance in long-term illnesses and rehabilitation and are involved in discharge planning.
