TEAM
Magnolia Regional Health Center participates in CMS TEAM (Transforming Episode Accountability Model). TEAM is a CMS initiative designed to improve care coordination and outcomes for Medicare patients undergoing certain surgical procedures. For more information you can visit cms.gov.
Surgical procedures included in TEAM:
- Lower extremity joint replacement (LEJR)
- Surgical hip/femur fracture treatment
- Spinal fusion
- Coronary artery bypass graft (CABG)
- Major bowel procedures
Care Coordination Commitment
As part of the TEAM commitment, Magnolia Regional Health Center will help improve patient experience with the coordination of care from the time of surgery through 30 days post-discharge, including referrals to primary care for continuity and long-term health outcomes.

Financial Transparency
TEAM uses an episode-based payment model that affects how providers are reimbursed and will not reflect changes to Medicare benefits or coverage for the patient. More information can be reviewed here.
Quality and Accountability
Through TEAM, Magnolia Regional Health Center will be measured on the quality of patient outcomes and experiences using three quality measures already reported under the Hospital Inpatient Quality Reporting (IRQ) and/or Hospital-Acquired Condition (HAC) Reduction programs.
